
Group Mentoring Outcome Survey Template

This survey template helps organisations measure how young people change during participation in group mentoring. It is designed to be used alongside the Group Mentoring Outcomes Framework and provides a structured set of questions for tracking progress before, during, and after group mentoring delivery.
The survey focuses on the main areas of change associated with group mentoring, including emotional regulation, peer interaction, participation in learning or group activities, decision making, confidence, school engagement, and trusted adult relationships. It includes a core set of 12 questions that can be used across most group mentoring programmes, with optional additional questions focused on reflection, perspective-taking, conflict, group contribution, and trust between young people.
The template is intended to be adapted before use. Organisations can simplify the wording for different age groups, shorten the question set, change the response scale, or use a practitioner-supported format where this would make the survey easier and more reliable for young people to complete. It can be completed at the start and end of group mentoring, and may also be repeated during delivery so that progress is still captured if a young person leaves before the programme ends.
Used with mentoring records, practitioner observation, and reflective discussion, the survey supports consistent evaluation of young people’s progress and helps organisations report how group mentoring contributes to emotional, relational, behavioural, and participatory change over time.
